Parents of players heading to CYC Chicago must complete the required fields below. Additionally, parents must upload the player's birth certificate, a photo of the player taken within the last twelve months, along with a signed CYC Waiver for Players and a CYC Concussion Code of Conduct for Players. If you have multiple players traveling to CYC Chicago you must fill out this form and upload the required documents for each of your players. 

 

Coaches traveling to CYC Chicago must fill out the form, upload a signed CYC Concussion Code of Conduct for Coaches, and a photo of themselves taken within the last twelve months.

Please download the relevant PDF(s), print and complete that, scan it (using an app like Genius Scan or taking a photo of it with your phone), then upload it when completing the form below.  Submitting the form and uploading the documents must be done at the same time in one sitting.  All fields and uploads are required for players.
