Parents of players heading to CYC Philadelphia must complete the required fields below. Additionally, parents must upload the player's birth certificate, a photo of the player taken within the last twelve months (passport style), along with a signed CYC Permission and Waiver for Players and a CYC Concussion Code of Conduct for Players. If you have multiple players traveling to CYC Philadelphia you must fill out this form and upload the required documents for each of your players.  PLEASE NOTE: IF YOUR PLAYER PLAYED IN CYC CHICAGO 2025, YOU DO NOT HAVE TO UPLOAD THEIR BIRTH CERTIFICATE.

 

Coaches traveling to CYC Philadelphia must fill out the form, upload a signed CYC Concussion Code of Conduct for Coaches, and a photo of themselves (passport style) taken within the last twelve months.

Please download the relevant PDF(s), print and complete that, scan it (using an app like Genius Scan or taking a photo of it with your phone), then upload it when completing the form below.  Submitting the form and uploading the documents must be done at the same time in one sitting.  All fields and uploads are required for players.
